This form is only for the use by the persons listed below in connection with the 12.125% Senior Secured Notes due 2018 (the "Notes") of Goodman Networks Incorporated ("Goodman"). Please use this form to obtain access to the secure website containing confidential information to be provided under the Indenture governing the Notes (the "Indenture"). All capitalized terms used and not defined herein shall have the meaning as set forth in the Indenture.
